Short/long sizes drive the "room for more" estimate. For Medicare patients, the long visit that earns a 99292 is 104 min.
Coding logic: <30 min = not critical care (use appropriate E/M). CPT: 99291 for 30–74 min; first 99292 at 75 min, then one more per additional 30 (105, 135…). Medicare: 99291 for 30–103 min; first 99292 only at 104 min, then 134, 164…. Excludes time on separately billable procedures. Commercial payers may follow either rule; check the payer's policy.
